How Membership Relates to Medical Care

Membership is a non-clinical service model that supports access, coordination, and time-based aspects of care.

• Membership does not replace health insurance
• Medical visits, procedures, and medications are billed to insurance when applicable
• Membership fees are separate from medical services

  Insurance functions as it normally would under your plan. Patients remain responsible for applicable copays, deductibles, or coinsurance. Membership fees are separate from insurance and are not billed to or reimbursed by insurance.

Membership Investment

Membership is offered on an annual basis, typically ranging from $1,900–$2,400, depending on the level of care and support appropriate for each patient.

Getting Started

We aim to make the process straightforward and supportive from the first point of contact.

Initial Conversation

Care begins with a phone call to our office. this conversation, we will:

• Answer initial questions
• Discuss your condition at a high level
• Help determine whether an in-depth evaluation would be appropriate
• Outline next steps

Scheduling

If appropriate, we will arrange an initial visit at a time that allows for a thorough evaluation.

Your First Visit

Your first appointment is typically a 90-minute comprehensive evaluation, focused on understanding your history and current concerns.

During this visit, we will:

• Review your medical history
• Discuss prior treatments
• Evaluate your condition in detail
• Begin developing an individualized treatment plan

Ongoing Care

Follow-up visits are typically 30 minutes and are designed to support continuity and thoughtful adjustment of your care plan over time.

Before Your First Visit

To ensure a productive first appointment:

• A referral from your treating provider may be requested
• Relevant medical records are reviewed in advance when available
• Please bring:

• A current medication list
• Completed intake paperwork
